Applying at Pilgrim Manor Minimize

After you have toured Pilgrim Manor and have decided to continue your journey with us, please contact our admissions office at (616) 940-5551. Our staff are ready to assist you with moving into Pilgrim Manor.

Admission Checklists Minimize

For Catered, Assisted, and Transitional Living

  • Completed, signed, and approved Admission Application, including the $500 application fee.
  • Physician completed and signed History and Physical form (dated no more than 5 days prior to move-in), including the list of all medications and dosages.
  • Copy of Chest X-ray report (dated no more than 90 days prior to move-in)
  • In-person medical review completed by Pilgrim Manor's Clinical Nurse Liaison
  • Signed Accommodation Acceptance form
  • Signed Pilgrim Manor admission contract and attachments
  • $2500 admission fee
  • Copies of all medical insurance cards, social security card, ambulance contract, ambulance contract card, etc.
  • Copies of current Power of Attorney papers, Patient Advocate forms and living wills, etc.

For Skilled Nursing

  • Completed, signed, and approved Admission Application
  • Physician completed and signed History and Physical form (dated no more than 5 days prior to move-in), including the list of all medications and dosages.
  • Copy of Chest X-ray report (dated no more than 90 days prior to move-in
  • In-person medical review completed by Pilgrim Manor's Clinical Nurse Liaison
